Aftercare Program
Our Aftercare Program supports clients living in the greater Charlotte area who have completed any of our residential programs within the last 3 years. The Aftercare Coordinator conducts home visits and helps clients maintain a safe home environment for their children and healthy relationships. The Aftercare Coordinator provides support, counseling, and resources for the healthy development of both the child and mother and helps families maintain stability, navigate resources and work towards their goals.
Objectives & Outcomes
Our goal is to provide counseling and continued care for families to sustain health and stability.
We work to ensure:
- Infants are current on immunizations
- Children reach their developmental milestones
- Participants have no substantiated cases of abuse or neglect
- Parents are able to pay their bills and maintain housing
Services & Approach
Prior to discharge from our residential programs, we create a plan with clients to best continue serving them once they are living independently.
During home visits, the social worker addresses the following topics:
- Child Development (Ages & Stages)
- Education and Career Support
- Parenting Skills
- Social and Life Skills Development
- Healthcare and Immunizations
- Housing/ Furniture Resources
- Repeat Pregnancy Prevention
- Substance and Alcohol Use Awareness
- Sexually Transmitted Disease Awareness
- Financial Literacy Support
